What is complex care?

Complex care, sometimes known as continuing healthcare, is specialist care provided to people with health or social care needs; giving support to enhance their quality of life, enable them to reach their full potential, and live independent and fulfilling lives.

Our complex care packages:

Health and Social Care Services is a specialist provider of home care services for people with complex care needs. Therefore, we recognise that when you have complex care needs it can feel like you have lost control of your own life. With our complex care services, we wish to give you that control back.

Our care packages are built by you and are designed to meet your needs. The care delivered can be at home, at work, on holiday, at university or wherever you need it to be. We can be engaged at any stage of your journey, from the first point of discharge to home and onwards.

We can provide:

  • 1:1 or 2:1 24-hour complex care or nurse led support.
  • Specialist bespoke complex care packages.
  • Waking night or sleeping support.
  • University, work or educational support.

We will support you in coming to terms with your injury or diagnosis as much as we possibly can, this support can be in terms of physical, psychological or social support. We welcome any complex care enquiries and will be honest in what we can and cannot deliver. From the initial assessment, always carried out by an experienced registered nurse, we will start to design your complex care package with you. Considering your likes, dislikes, wishes and aspirations.

Building YOUR complex care package:

Throughout the building of your complex care package and the recruitment of the team we will keep you as informed as you would like to be. You are welcome to interview with us if you wish. Open and honest communication being key to a successful relationship. Once the team is in place, fully checked in line with CQC requirements and our professional duty of care, we will then provide any training required and ongoing support for the team and, if required, your family members. Training will be classroom and home based to ensure it is fully personalised to your needs. We will continue to monitor the care delivery throughout and ensure we are on hand to help when needed. We can visit you as often or as little as you like, using our IT system to monitor from a distance when you would rather us be discreet.

In addition to supporting you and your team, we can also provide evidence on a regular basis, or as and when required, in any format for case managers or commissioners.

Referrals

Referrals for Health and Social Care Services Limited are welcomed from Case Management teams, Local Authorities, NHS Trusts and Clinical Commissioning Groups, Charities and directly from home and family.

Upon referral, our specialist nurses will contact you within 48 working hours to discuss current availability and suitability. We will then arrange for a care needs and outcomes assessment to be undertaken. These will act as our baseline assessment and will help to establish what your needs are and how we can best meet your wishes and aspirations. This assessment will also then be used as a starting point for the formation of personalised care plans and risk assessments.

What is continuing healthcare / complex care & who is eligible?

Clients receiving complex care have substantial and ongoing healthcare needs. These can be the result of chronic illness, disabilities or following hospital treatment. Complex care is sometimes referred to as long-term care or continuing care. You may be eligible for continuing health care if you meet the NHS criteria.
