HASCS was founded in 2017, born with the vision to create a truly nurse-led service. We provide care for people of all ages living with spinal injuries, physical disabilities and life-limiting conditions with a complex care plan tailored specifically for their care needs and personal preferences.

What is complex care and how can HASCS help?

It’s tailored to people whose needs go beyond what standard home care or assisted living usually covers. With complex care, a multidisciplinary team work together to build a care plan that fits the individual, not just the condition.

We understand people have unique requirements, so we take the time to get to know our service users and their families, to ensure we fully understand not only their clinical requirements, but equally their individual interests, hobbies, likes and dislikes.

Yes, you can continue enjoying holidays, safe in the knowledge that you will have the right support. At HASCS, we enable individuals to lead fulfilling lives while ensuring their safety and well-being.

As a team, our vision is to be solution focused and have a ‘can do’ attitude. In practice, this means that if there is a holiday you wish to go on, we will do our best to make this happen. We pride ourselves on being honest in what we can and cannot deliver.

Yes, you can continue socialising and doing the things you love with the right support in place.

As well as holidays, we can support day-to-day social engagement and activities. We facilitate and encourage social interaction through trips to museums, theatres, or community events. Support for hobbies such as sports, arts, or dining out, as well as access to specialist clubs or day centres, can all be facilitated.

How will my needs be assessed and who will care for me?

At HASCS, we’re committed to providing specialist, bespoke care that feels genuinely personal. To do this well, we start every new relationship with a structured, clinically led meet‑and‑greet, designed to help us truly understand you and build a foundation of trust from day one.

The meet‑and‑greet is your first formal point of contact with us. It gives our clinical team the chance to get to know you, learn about your needs, and explain how our care model works. It’s also a great opportunity for you to ask questions and for us to set clear, shared expectations. We believe that high‑quality, sustainable care begins with open, honest conversations.

Each session is led by one of our senior clinical colleagues. During the visit, we’ll explore the things that matter most to you, your routines, communication preferences, any specific clinical considerations, how you’d like your care team to work with you, and anything else that could help shape your personalised care plan.

Throughout this process, we stay in close contact with families, Integrated Care Board (ICB) representatives, and any other key stakeholders. Clear and consistent communication helps make the journey from assessment to mobilisation as smooth as possible, ensuring you can move into care with confidence and clarity.

By keeping clinical leadership, transparency, and person‑centred values at the heart of every step, we ensure that every individual entering our services receives safe, consistent and exceptional care.

How do I arrange complex care for myself or a loved one?

Many clients are referred to us from their Continuing Healthcare team or another department while in the hospital. Our dedicated team works closely with healthcare professionals to mobilise care plans allowing for a safe and supported return home.

However, if you have an existing Personal Health Budget (PHB) in place or you have not yet been referred and you or a loved one requires complex care services, then you can reach out directly to our team to discuss what services you may require.

We welcome any complex care enquiries and will be honest in what we can and cannot deliver. From the initial assessment, carried out by an experienced registered nurse, we will start to design your complex care package with you, always considering your likes, dislikes, wishes and aspiration.

Speak to our care professionals for comprehensive care solutions tailored to your needs

We welcome referrals from case management teams, local authorities, NHS Trusts and integrated care boards, charities as well as directly from the home and family. Upon referral, our team will contact you, where they can talk you through the entire process and guide you through each stage, as well as answering any questions you may have.
