We have comprehensive experience of supporting children and young adults with long term and complex conditions. We pride ourselves on supporting the child, their siblings and the family, coordinating the clinical care in their own home.

We work closely with families in our care whilst working inclusively with the wider health, social and educational teams.

Offering control back to the family we ensure we are:

  • Creative thinking to meet aspirations and wishes.

By building support around the whole family we can offer back the security and safety required to allow the child and their family to enjoy living at home.

We are fully clinically led and managed by registered nurses including Paediatric and School Nurses – allowing us to confidently support a wide range of needs, including Cerebral Palsy, Congenital and Neurological conditions and learning disabilities.

 

Transitioning to Adult Care

We understand that supporting young people through their transition from children’s care to adult care is key to a successful and fulfilling experience in their early adult life.

The transition from children’s services to adults’ services is often very challenging for young people and their families – it combines a change of services and professionals at the very time when they are also experiencing other changes to their life.

We support the wider multidisciplinary team to plan these changes in advance.

With in-depth planning and a gradual transition, we ensure the transition will be as stress free as possible.

We can ensure there will be no gap in services, where someone reaching the age of 18 suddenly finds themselves without the quality of care and support they need.

Our service delivery is transparent to clients, families, associated healthcare professionals and commissioners. We fully embrace the use of information technology and have systems in place that allows all appropriate parties to have access to real time care delivery information.
